Sagging skin and jowls

Skin that has begun to soften along the jawline, under the chin or on the neck can often be firmed without surgery, gradually and modestly. Focused ultrasound (HIFU) and radiofrequency heat the deeper layers so they contract and build new collagen, and the change builds over two to three months. How much they help depends on how deep the laxity sits, which is what the doctor looks at first.

What may help

The short answer

Early softening responds to radiofrequency that firms gently over months; deeper laxity of the jawline and neck is a case for focused ultrasound, which lifts from the layers beneath. Neither uses needles, and both usually need no recovery time; some redness or tenderness for a few days is common.

Probably not that: Neither replaces surgery for significant laxity, and we will say so if that is the honest answer.

What it is, and why it happens

Sagging is laxity: tissue that used to hold its place begins to sit lower. Every layer of the face ages, from bone and ligaments to fat, muscle and skin, each at its own pace, and that pace differs from person to person and between ethnic backgrounds. Along the jaw, the short, elastic tissue that lets the jaw move freely beneath the skin lengthens with age; that slack, in the skin and the layer just below it, is what forms a jowl.

Two things can speed it up. Sun ages skin faster: in a trial that followed adults for four and a half years, those who used sunscreen every day showed no detectable increase in skin ageing. And losing a lot of weight, especially quickly, as can happen with weight-loss medicines, thins the fat of the face and loosens the skin, so the face and neck can sag.

For choosing care, what matters is how deep the change sits. Early softening, with contours still holding, suits gentle firming. A softer jawline and a looser neck suit treatment that reaches the deeper layers. Laxity that comes with crepey texture, enlarged pores or acne scars suits work within the skin. Sagging that is really lost volume, where the cheeks have hollowed and the face has dropped below them, is a question for injectables and a separate plan; marked laxity with folds of loose skin is a question for a surgeon.

What may help

Ultraformer MPT · Ultrasound lifting

Focused ultrasound (HIFU) places tiny points of heat at set depths, down to 4.5 mm in the SMAS layer, where the tissue contracts and builds new collagen. It suits a jawline that has softened, a neck that has loosened and early jowls. Usually one session every 12 to 18 months, or two or three spaced four to six weeks apart for advanced laxity; the change arrives at two to three months as collagen rebuilds. In a review of 17 studies the average improvement was moderate, and in a small study of this device for sagging eyelids, run with its maker, blinded doctors saw an improvement in about six in ten people. Downtime is usually none; some redness, mild swelling or tenderness for a few days is common.

Volnewmer · Radiofrequency tightening

Volnewmer is monopolar radiofrequency: waves of gentle heat reaching both layers of the dermis, up to 4 mm deep, while water cooling protects the surface. The gentler of our two tightening tools, it suits early jowls, soft contours and crepey skin, when the aim is firmness rather than lift. Usually one session every 12 to 18 months, or two spaced four to six weeks apart for more advanced laxity; firmness builds from the second to fourth week and peaks around two to three months. In a small randomised trial of this device, one session gave a modest lift at eight weeks. Downtime is usually none; mild redness and warmth for a few hours are common.

Sylfirm X · RF microneedling

Sylfirm X is radiofrequency microneedling: fine gold-plated needles release energy at set depths, from 0.3 to 4 mm, and its continuous mode is designed for tightening and collagen remodelling. It suits early laxity that comes with texture, enlarged pores or acne scarring, on all skin tones after assessment. Usually three to five sessions about four weeks apart; the main change shows at eight to twelve weeks, and remodelling can go on for months after. Downtime is usually minimal: pinkness and a sunburn-like feeling for a day or two; less often bruising, a breakout or a cold sore, and uncommonly darker patches, more likely on deeper skin tones and with sun.

What to be careful with

Expect firming, not a facelift. Results vary from person to person, and some people see little change; that is worth knowing before a course rather than after.

Filler does not tighten skin: it restores volume where volume has gone, which is a separate plan. If you have had filler or anti-wrinkle injections recently, the device sessions are simply planned a little apart, and the doctor sets the order.

What matters in Dubai

Dubai's sun is strong all year, so daily broad-spectrum SPF, reapplied when you are outdoors, belongs in any plan for firmer skin.

After focused ultrasound or radiofrequency, avoid saunas, hot yoga and strong sun for a week or two, which in summer means planning around the beach. Sylfirm X needs skin without a fresh tan or sunburn, so leave a gap after a holiday in the sun. If you are fasting in Ramadan, tell us when you book: an appointment after iftar may suit you better.

Focused ultrasound works beneath the surface rather than on pigment, and in a review of 17 HIFU studies none reported darker patches afterwards. Radiofrequency microneedling suits all skin tones after assessment, but any heat-based treatment can trigger pigment, so on deeper skin tones the settings start carefully and sun protection matters even more.

When to see a doctor first

A face that drops suddenly, especially on one side, is not sagging: a droop that appears over hours or days, with or without weakness in an arm or slurred speech, needs emergency care (call 998 in the UAE). Sagging that came with weight loss you can't explain, or with a new lump in the neck, is also for a doctor before anything cosmetic.

Before focused ultrasound or radiofrequency, the doctor asks about pregnancy, a pacemaker or other implanted device, metal implants in the area, skin infection or broken skin, recent filler or other procedures in the area, isotretinoin in the last six to twelve months before microneedling, keloid scars and conditions that slow healing, and works out how much of the change is laxity and how much is lost volume. Dr. Svetlana Tagaeva, the Medical Director, leads the clinic's medical work, energy-based treatments included, and Dr. Olga Ibrakova's work includes Ultraformer MPT and Sylfirm X.

Questions people ask

Does HIFU really lift the face?

It firms and lifts a little, gradually: the aim is a firmer jawline, not a different face. The change builds as collagen rebuilds and varies from person to person, and where sagging is marked it does not match surgery.

How long do the results last?

The skin goes on ageing, so firmness is kept up over time, usually with one focused-ultrasound or radiofrequency session every 12 to 18 months.

HIFU, radiofrequency or RF microneedling: which is right for me?

It depends on depth. Early softening and crepey skin usually suit radiofrequency (Volnewmer); a softer jawline and a looser neck suit focused ultrasound (Ultraformer MPT), which reaches deeper layers; laxity with texture, pores or scarring suits RF microneedling (Sylfirm X). The doctor may suggest one, a combination over time, or a different answer altogether.

Does it hurt?

Focused ultrasound is felt as warmth and brief deep pinches; across studies the average pain score was about 4 out of 10, and numbing cream is optional. Volnewmer feels like waves of warmth with a gentle vibration, and Sylfirm X is done after numbing cream, as a warm, prickling pass.

I'm losing weight with weight-loss injections. Should I wait?

Not necessarily for skin firming, which can start while your weight is still changing. Volume is different: it is usually judged once your weight has settled, so the face is not overfilled as it changes.

How far ahead of a wedding or Eid should I start?

Two to three months at the least, because the change builds slowly. A course of Sylfirm X needs longer, since its sessions are about four weeks apart and the main change follows two to three months after.

How much does skin tightening cost in Dubai?

We don't publish prices. The cost depends on the areas treated, such as the face, the neck or both, on the device that suits your laxity and on the number of sessions, which we set together at the consultation. Most people need one session of focused ultrasound or radiofrequency, while RF microneedling is usually a course of three to five. You know the full plan and its cost before you start, and we suggest only what your laxity calls for.

Where can I get skin tightening in Dubai, and how do I choose a clinic and doctor?

At CREAMÉ, 350 Al Wasl Road, Jumeirah 1, where Dr. Olga Ibrakova's work includes Ultraformer MPT and Sylfirm X, Dr. Daria Banina's includes both of these and Volnewmer, and Dr. Svetlana Tagaeva, the Medical Director, leads the clinic's medical work. Wherever you go, look for a doctor licensed by the Dubai Health Authority who examines your face and neck before naming a device, explains the risks, and tells you when lost volume or surgery is the honest answer instead.

Which areas of the face and neck can skin tightening treat, such as the jawline, jowls, under the chin or under the eyes?

It can firm a softened jawline, early jowls, the skin under the chin, a looser neck and crepey skin on the face; around and under the eyes it can firm crepiness, but it does not treat bags, puffiness or hollows. The doctor matches the device to each area.

Do you offer skin tightening for the body, such as loose skin on the stomach or arms after weight loss?

Not with focused ultrasound or radiofrequency, which we use on the face and neck. On the body, skin that has lost some tone after a pregnancy or a change in weight may respond to icoone, gentle suction with laser and LED light, offered as wellness bodywork over a course of usually 10 to 15 sessions; the change is gradual and varies from body to body. Folds of loose skin after a large weight loss are beyond what bodywork can change, and a question for a surgeon.

Is laser skin tightening the same as radiofrequency or ultrasound (HIFU)?

No, though people often call any skin tightening “laser”. Our tightening tools don't use light: Ultraformer MPT focuses ultrasound into points of heat down to the SMAS layer at 4.5 mm, and Volnewmer and Sylfirm X deliver radiofrequency, an electric current, into the skin. Working beneath the surface rather than on pigment, they usually suit a wide range of skin tones after assessment, though any heat can leave darker patches, so settings start carefully. MOXI, our resurfacing laser, is for tone and texture.

How many sessions of skin tightening will I need?

For most people, one session of focused ultrasound or radiofrequency; more advanced laxity may take two or three ultrasound sessions, or two of radiofrequency, four to six weeks apart. RF microneedling is usually a course of three to five sessions, about four weeks apart. The change builds over two to three months as new collagen forms.

Does skin tightening really have little or no downtime, and what are the side effects?

Usually, yes: most people go back to their day straight away, though RF microneedling leaves a day or two of pinkness. Common and short-lived: redness, mild swelling or tenderness for a few days after focused ultrasound, and redness and warmth for a few hours after radiofrequency. Less often: small bruises, numbness or tingling, darker patches, a breakout or cold sore after microneedling, or weakness of a facial muscle, usually temporary. Rarely: burns or blisters, a small hollow where fat has shrunk, or a scar.

What aftercare helps after skin tightening on the face?

Wash gently and use a plain moisturiser while any tenderness settles. After Volnewmer, make-up can go back on straight away; after RF microneedling, leave it for a few days, along with acids, retinoids and scrubs. For a week or two, skip saunas, hot yoga and strong sun, wear SPF every day, and leave a gap, which we plan with you, before filler or anti-wrinkle injections in the same area. Message us straight away about blistering, pain that gets worse, numbness that lasts or weakness when you move your face.

Are there skin tightening injections for the face, and what are their side effects?

Not in the way focused ultrasound or radiofrequency tightens, but biostimulators can add gradual firmness: biodegradable particles prompt your own collagen over two to three months, and the effect usually lasts around two years. They cannot simply be dissolved, so the doctor decides carefully whom they suit; filler restores volume rather than tightening. Swelling, tenderness and bruising are common and usually settle within one to two weeks; less often there are lumps, and a blocked blood vessel is rare but serious.

Can I have skin tightening during or after pregnancy?

Not during pregnancy or while breastfeeding: focused ultrasound, radiofrequency, RF microneedling and icoone all wait until afterwards. After a birth, part of a softer tummy is often the two long muscles down its middle, which usually come back together by about eight weeks; a gap still obvious then is worth showing a doctor. Once you have finished breastfeeding, face and neck treatments can be planned as usual, and icoone may help body skin that has lost some tone; after a caesarean, we wait until the scar has fully healed.
